Patient Navigation
Patient navigation at HopeCentral is designed to support families as they seek to find the best care for their child.
A Patient Navigator supports families in the navigation of your child’s diagnosis and determine the next steps. At HopeCentral, we are able to guide families through the complex medical referral system and overcome barriers to accessing specialized care.
